Step 4: Submit your full name and email

Scroll the appearing page down, then enter your full name and a valid email address. Make sure you have “The subject of the request” selected in the dropdown, check the agreement to receive opt-out communications, solve the CAPTCHA, and click “Send Request.”

Pro tip: We recommend that you create a separate email address that doesn’t disclose your personal information.
